Please do not send medication to school in lunch boxes, pockets, or pocketbooks. It is extremely important that you notify the nurse if your child is on long time medication at home. All students registering in a Connecticut public school for the first time (from out of state) must have a completed State Department of Health Assessment Record form  (physical) and immunization records in the school BEFORE they are able to attend.</div> <div> Students registering from within Connecticut do not need physical examinations, but must have immunization records.</div> </tD> </tR> <tR> <tD VALIGN=TOP HEIGHT= 196 WIDTH="496"><div> <B>Emergency Care Cards</B></div> <div> Your child's emergency care card is kept on file in the school office so that you can be reached in case of an accident or illness during the school day. Please be sure to list your business phone numbers. The friends you list should be available to take your child home if he/she becomes ill.</div> <div> Please phone if any changes occur during the school year (i.e. a job change). Many doctors will not treat a child's injury without parental consent, so it is important that we be able to reach you in an emergency.</div> </tD> </tR> </TABLE></div> <bR> </td> </tr></table></body>
