Medications given in school requires written consent of parents and/or guardians and written directions from a doctor (there is a special form available for this).
A parent or guardian must bring the medication to the school nurse with the written consent/directions. Please do not send medication to school in lunch boxes, pockets, or pocketbooks. It is extremely important that you notify the nurse if your child is on long time medication at home. All students registering in a Connecticut public school for the first time (from out of state) must have a completed State Department of Health Assessment Record form (physical) and immunization records in the school BEFORE they are able to attend.
Students registering from within Connecticut do not need physical examinations, but must have immunization records.
